Camp Waivers
Several waivers are required to attend CI camp. Review the waivers below and complete all that are applicable to you. To print, click on the waiver button, then use the print feature of your web browser to print off the number of copies that you need. When completed, please scan the completed forms to a pdf file and email to:
If your printer does not have a scanner feature, you may be able to scan the forms with your phone or take photos of the forms with your phone. If you have any questions about or difficulties with the waivers, please contact Julie Schlager at the above email address.
Please make sure to send your completed waivers at least a week before the start of the Camp.
Listen Foundation Family Waiver.
One waiver should be completed for the entire family. Click on the link and print your waiver.
YMCA of the Rockies Waiver
A YMCA of the Rockies waiver must be completed for each family member. Click on the link and print the necessary number of waivers.
Horseback and Pony Riding Waiver
A horseback and pony riding waiver must be filled out for each rider. Click on the link and print off the needed number of waivers.
Hayback Ride Waiver
A hayback ride waiver must be filled out for each wagon rider. Click on the link and print the needed number of waivers.
Volunteer Waiver
A volunteer waiver must be filled out for each volunteer. This does not need to be completed until your volunteer application has been approved. Click on the link and print your waiver.
